"Fatal Error" details the shuttle flight to Belfast of "Flight 92" in 1989.  A vibration caused by a fan blade breakdown at 28,000 feet resulted in the pilots following the manual for a shut down drill - but closing the good engine.

Having seemingly stopped the vibration problem, they assumed they had selected the damaged engine and diverted to a closer airport. 
In sight of the airport, they lost their remaining engine and had to crash land on the runway.  47 people lost their lives.
